Our Profile

Providing person-centred care to individuals with dementia, physical disability, sensory impairment, learning disability or autism, but not in a specialised way, rather providing personal care for their personalised needs.

The individual is visited at various times of the day as per their preference, or in some cases, a 24-hour per day.

In the morning, the individual might have a visit to support with washing, bathing or with a shower, dressing/undressing, incontinence care, toiletting, breakfast and support with medication, support with transfer and mobility and tidying up used areas.


 

At lunch time,  support with incontinence care, toiletting, lunch, medication, and tidying up used areas.

​

In the evening,  support with incontinence care, toiletting, tea, meal preparation, medication, and tidying up used areas.

​

In the night, support with incontinence care, toiletting, tea, washing, changing to night wear, medication, and tidying up used areas.

​

Other support such as domestic, shopping, laundry, companionship, errands.

To develop and implement personalised care plans that reflect the individual’s physical, emotional, social, and cultural needs, particularly tailored for those living with dementia or physical disabilities.

Ensure Safety and Well-being:

To safeguard the health, well-being, and safety of all service users by adhering to the highest standards of care, risk management, and regulatory compliance.

Support Inclusion and Engagement:

To encourage social inclusion and meaningful engagement, reduce isolation and promote mental and emotional well-being through consistent and compassionate care relationships.

 

Continuously Improve Quality of Care:

To regularly review and improve the quality of our services through service user feedback, staff development, audits, and compliance with CQC standards and best practices.

Empowering Staff Through Training and Support:

To ensure that all care staff are trained, supported, and equipped to provide expert care—especially in understanding the needs of individuals with physical disabilities and those affected by dementia.

Promote Partnership and Collaboration:

To work collaboratively with families, health professionals, and local authorities to ensure continuity of care and effective communication across all stakeholders.
